Community Integration Services (CIS)
Adult Mental Health
A strength-based, person-focused service. Case managers assist and empower the individual to augment quality of life and acquire independence. It is a process that collaboratively reviews and supports natural strengths and assesses and aligns community resources. The case manager will coordinate referrals and advocate for access to resources as well as monitor service provision and regularly review progress made toward achieving goals. Success in this program will require active participation by the consumer and, if applicable, the guardian.
Adult Community Case Management (ACCM)
Adult Developmental Services
ACCM provides case management services to individuals, 18 years and older, with a diagnosis of Intellectual Disabilities, Autism, and/or other Developments Disabilities, who have been found eligible by the Department of Health and Human Services Office of Aging and Disability Services (OADS). ACCM works to assist adults with gaining access to education, employment, housing, social opportunities, and healthcare, as well as many other resources that the community has to offer. This work is guided by a Person-Centered Plan (PCP), which clients create in collaboration with their team of providers to highlight personal needs and desires. At the core of all our efforts are the principles of community inclusion and personal independence, and we tenaciously advocate for our clients so that they may reach optimal functioning in both of these areas.
